I am very pleased with both it and the Milwaukee drill. I have had it come out of the drill twice and that causes me concern and I hope to figure out how to avoid this trend as it does cause a moment of panic but is really a non issue and but a minor inconvenience.
I haven't had any issues with mine coming out yet. Are you sure you're lining up the flats on the shaft with the chuck?

The first time, I am not sure as it was last year while it was still new to me.
This past weekend for the 2nd time...I can say that yes, I am sure it was on the flats. Either the chuck was not tight enough or I pulled up to clear the hole with too much weight/shavings in the hole.
I will pay more attention to this in the future.
Once a year is not a big deal but it does scare you briefly.

Not sure what kind of drill you are using, but on the Milwaukee drill, if memory serves, once you tighten the chuck on the drill, you grab the chuck and turn it, backwards?? I think just a little bit and there is a click and it is locked.  I may have that backwards, I don't have the drill with me right now, but someone who has it can probably chime in.  Most drills have something similar.

That's correct, the Milwaukee has an interesting chuck that I haven't seen before, it clicks when it's tight enough. My Makita didn't have any feedback like that. The Milwaukee hasn't loosened up on me yet and I've used it a lot.
