Yeah, same here.  I now tighten it by hand (without running the drill) to minimize the damage.

It appears you can order replacement parts right from ardisam.com, which I have not yet done.

Tremendus amount of stress on adapters. The first one i made was for 3/8 chuck drill. When I put the new rigid on it she twisted rite off. Lol made a 3 sided top on it for 1/2inch and its fine. If it loosens up it will chew it up though.

I switched to the quick connect system from Deep Freeze and don't have any more issues. Highly recommend.
